Drug-induced hepatotoxicity leads to nearly 10% of all cases of acute hepatitis and more than 50% cases of liver failure [18]. It is one of the common reasons for the withdrawal of medications from the market and modification of use [19]. It can be either type A (predictable), dose-related and short latent period in days, or type B (idiosyncratic), dose-independent, unpredictable, and variable latency [20,21]. Based on population-based studies, drug-induced liver injury incidence varies between 13.9 and 19.1 cases per 100,000 people per year [22,23]. Patients have either hepatocellular injury (three times upper limit of transaminase in comparison to ALP), cholestatic injury (three times increase in ALP comparison to transaminase), or mixed pattern (where both ALP and aminotransferase are three times upper limit) [24-26]. Most patients improve spontaneously after the removal of the offending drug. If acute liver failure (ALF) is suspected, early liver transplant referral is important due to high ALF mortality [25,27]. From the spontaneous reports from patients who received Pfizer/BioNTech BNT162b2 mRNA in the UK between 9/12/20 and 26/05/2021, there are reports of 45 patients having abnormal liver function analysis and three patients having drug-induced liver injury [28].

In this case, the review of medications and history did not reveal any other reason for hepatotoxicity. She also denied the use of any over-the-counter medications or supplements. Although it is rare with vaccination, the COVID-19 vaccine is likely the cause of hepatotoxicity in our patient based on a diagnosis of exclusion. In this case, the patient had a cholestatic pattern with elevated ALP and bilirubin with mild elevation in the transaminases.

Pfizer/BioNTech BNT162b2 mRNA trial included only 0.6% (217/37,706) patients with liver disease. Among patients with liver disease, 214 were with mild liver disease and only three with moderate to severe liver disease. This patient has underlying fatty liver disease. It is unclear if that was a likely risk factor for hepatotoxicity in this case [5].  Although only a small number were included in trials for Pfizer/BioNTech BNT162b2 mRNA, Moderna mRNA-1273, and the AstraZeneca/University of Oxford ChAdOx1-nCoV-19 chimpanzee adenovirus vector vaccine, both the American Association for Study of Liver Diseases and European Association for the Study of Liver recommend vaccination against SARS-COV-2 with these highly effective and safe vaccines, given a greater risk of health consequences from SARS-COV-2 infection in these patients [29,30].

Hepatotoxicity can occur with vaccines, even though it is more common with prescription and nonprescription drugs. So, the clinician should be watchful in patients showing clinical signs and symptoms after a vaccine.

Conclusions

COVID-19 Policies Wrecked Public School Enrollment and Student Outcomes

After the historic one-year enrollment drop of 2.5 percent in the 2020-21 school year, public K-12 attendance has stubbornly refused to bounce back. Two new studies further indicate that the biggest two-year declines correlate strongly with the most restrictionist school-opening policies, particularly in Democratic-controlled big cities.

The American Enterprise Institute (AEI) released a report Wednesday showing that nationwide enrollment in government-run public schools in the 46 states under review declined an additional 0.2 percent in 2021-22, but with a measurable split at the extreme ends of pandemic-related school policies. The most remote school districts lost an additional 1.2 percent enrollment on average in 2021-22, contributing to a two-year decline of 4.4 percent; while the most open districts rebounded by 0.9 percent this year and have lost just 1.1 percent overall since COVID-19 hit.

The divergence in pandemic school policies has never correlated with comparative rates of campus spread, but rather with the comparative concentration of Democratic voters and locally powerful teachers unions. These factors, along with high costs of living and historically low rates of immigration, have combined to make the past two years an enrollment wipeout in the biggest cities.

New York City, the largest district in the nation, has lost a staggering 9.5 percent of students since the onset of COVID-19. Los Angeles Unified, the second largest, where unions have had particular success in getting most every restriction and compensation they wanted, saw the student body shrink by 8.1 percent. School Superintendent Alberto Carvalho, in an article earlier this month at The 74 Million, described the confluence of the enrollment drop and the drying out of the nearly $200 billion in emergency federal COVID relief funds to K-12 schools as potentially "Armageddon."

Unless the long-term trend of Angelenos fleeing the public school system somehow reverses, Carvalho warned, "It's going to be a hurricane of massive proportions."

The only two of the country's top 10 school districts that haven't lost population since the pandemic hit are both in Florida (Orange and Hillsborough counties), where Gov. Ron DeSantis ordered schools open by edict in the fall of 2020. Of the 46 states in AEI's study, only four added students to their public schools: The heavily Republican South Dakota, Utah, North Dakota, and Idaho. The six states that lost between 0 percent and 1 percent during that time, including Florida and Texas, are also GOP strongholds.

"How schools operated affected family decisions," AEI Education Policy Studies Deputy Director Nat Malkus told the Washington Post.

The school tracking service Burbio earlier this month released a study of 2021-22 enrollment trends in 40 states, breaking out data among the four main geographic designations used by the National Center for Education Statistics: Rural, Town, Suburban, and City. Notably, only the "city" category saw a decline this year:

"The effects of the sharp, recent enrollment declines may be long-lived," Stanford University Education Professor Thomas Dee told The 74 Million. "The fiscal consequences will remain for some while."

K-12 spending amounts to around 20 percent of all state and local government spending. If the customer base for this freely offered product continues to reject it in favor of more expensive options, not only will education budgets (which are usually tied to enrollment numbers) get slashed, the political enthusiasm for paying the price tag via taxation will likely wither.

America was a global outlier in the amount of closures and restrictions imposed on public schools. The Centers for Disease Control and Prevention (CDC) in particular played a key institutional role in mixing messages and producing senseless over-caution. Remote learning didn't just repel students, it consigned the ones who remained to staggering amounts of learning loss. We do not yet understand the full extent of the hit, but what public education decision makers did to public schools the past two years will likely go down as one of the most flagrant and impactful acts of institutional self-harm in the 21st century.

National Institutes of Health acting director Lawrence Tabak confirmed to lawmakers Wednesday that US health officials concealed early genomic sequences of COVID-19 at the request of Chinese scientists — but insisted the data remains on file.

Tabak told a House Appropriations subcommittee that the NIH “eliminated from public view” the data from the pandemic epicenter in Wuhan, China, before adding that researchers can still access it via an archaic “tape drive.”

Vanity Fair recently reported that the information was hidden in response to a request from Chinese scientists, despite potentially resolving whether the virus leaked from the Wuhan Institute of Virology or passed naturally from animals to humans.

Rep. Jaime Herrera Beutler (R-Wash.) asked Tabek to explain why US officials would comply with such a request

“There’s no question that the communication that we had about the sequence archive — Sequence Read Archive — could have been improved. I freely admit that,” Tabek said. “If I may, the archive never deleted the sequence, it just did not make it available for interrogation.”

Children are turning up in doctors' clinics infected with as many as three different types of viruses, in what experts believe is the result of their immune systems being weakened from two years of COVID lockdowns and mask-wearing.

Medical staff have come to expect a surge in cases of flu and severe colds during the winter. 

But they are reporting that there is not the usual downturn as summer approaches - and they suspect it could be due to the strict pandemic practices.

Furthermore, some of common strains of the flu appear to have disappeared, flummoxing scientists.

Thomas Murray, an infection-control expert and associate professor of pediatrics at Yale, told The Washington Post on Monday that his team was seeing children with combinations of seven common viruses - adenovirus, rhinovirus, respiratory syncytial virus (RSV), human metapneumovirus, influenza and parainfluenza, as well as the coronavirus

Some children were admitted with two viruses and a few with three, he said.

'That's not typical for any time of year and certainly not typical in May and June,' he said.

CDC data obtained by DailyMail.com showed lower overall levels of influenza infections among young children - but an abnormal surge starting several weeks ago during the beginning of the summer months, normally a dead period for respiratory infections.

Other strange patterns have emerged.

The rhinovirus, known as the common cold, is normally not severe enough to send people to hospital - but now it is.

RSV normally tapers off in the warmer weather, as does the influenza, but they have not.

And the Yamagata strain of flu has not been seen since early 2020 - which researchers say could because it is extinct, or perhaps just dormant and waiting for the right moment to return.

'It's a massive natural experiment,' said Michael Mina, an epidemiologist and chief science officer at the digital health platform eMed, told the Post. 

Mina added that the shift in what time of year Americans are seeing infections is likely due to the population's lack of exposure to once-common viruses - making us vulnerable when they return.

'When you have a lot of people who don't have immunity, the impact of the season is less. It's like free rein,' he said.

The virus can therefore 'overcome seasonal barriers.' 

Peter Hotez, a molecular virologist and dean for the National School of Tropical Medicine at Baylor College of Medicine in Houston, agreed that the norms are shifting, and the seasonal patterns no longer apply. 

'You would see a child with a febrile illness, and think, 'What time of the year is it?' ' he said.

The shifts are also making hospitals rethink their approach to RSV - a common virus that hospitalizes about 60,000 children under five each year. It can create deadly lung infections in particularly vulnerable youngsters. 

Treatment is with monthly doses of a monoclonal antibody, which is normally only available from November to February.

Now concerned scientists are tracking the virus carefully, in case they suddenly need to obtain the drug.

Ellen Foxman, an immunobiologist at the Yale School of Medicine, whose research explores why viruses can make one person very sick but leave another relatively unharmed, said that babies born during the pandemic are likely to be of great interest to scientists.

'Those kids did not have infection at a crucial time of lung development,' she said.

Foxman added that much has been learned, by the population as a whole as well as scientists, about viruses and how to prevent infection over the last few years.

'We need to carry some of the lessons we learned forward,' she said.

He is not considered a close contact to the president and has only mild symptoms.

 

Fauci, director of the National Institute of Allergy and Infectious Diseases (NIAID), part of the National Institutes of Health, and Chief Medical Advisor to President Joe Biden, tested positive for COVID-19 on a rapid antigen test.

 

He is fully vaccinated and has been boosted twice.

Federal COVID Aid to States and Localities Cost $855,000 Per Job Saved

A massive federal bailout of state and local governments during the pandemic had a negligible effect on overall employment levels despite costing taxpayers an estimated $855,000 per job saved.

That's the bottom line of a new National Bureau of Economic Research working paper published this week. In the paper, a trio of researchers attempted to determine the effectiveness of nearly $1 trillion in pandemic-era aid distributed to state and local governments—the majority of which was delivered as part of the $1.9 trillion American Rescue Plan passed in March 2021.

Despite the eye-watering price tag, however, the stimulus spending had only "a modest impact on government employment and has not translated into detectable gains for private businesses or for states' overall economic recoveries," conclude University of California, San Diego economists Jeffrey Clemens and Philip Hoxie and American Enterprise Institute senior fellow Stan Veuger, the paper's three authors.

The aid to states and local governments did not meaningfully affect the broader economy either, the paper argues, in part because of other government responses to the pandemic. "Both voluntary and mandated limits on economic activity inhibit standard transmission mechanisms that can make fiscal stimulus effective under other circumstances," Clemens wrote on Twitter.

The case for bailing out state and local governments was always paper-thin. Despite fears of massive budget shortfalls due to a combination of government-imposed lockdowns and changing consumer behavior during the pandemic, those deficits mostly did not materialize. Before the American Rescue Plan passed, there was widespread skepticism about the proposed bailout, in part because three other pandemic-era spending bills had already sent about $360 billion in aid to states and localities.

But instead of stimulating the economy or filling budget holes that mostly didn't exist, the aid to states and local governments just left them flush with cash. As Reason has reported, some of that money was spent on unrelated things like money-losing, government-owned golf courses.

And while saving jobs or preventing layoffs was not the sole purpose of the American Rescue Plan's spending on state and local governments, that was and remains a centrally stated goal of the bailout. "These funds ensure that governments across the country have the flexibility they need to vaccinate their communities, keep schools open, support small businesses, prevent layoffs, and ensure a long-term recovery," Deputy Treasury Secretary Wally Adeyemo said in January of this year, as the Biden administration loosened some of the restrictions on how those funds could be used.

Even within the context of other federal emergency spending plans, the state and local bailout looks massively wasteful. Clemens, Hoxie, and Veuger point out that the $855,000 per-job figure dwarfs estimates of other emergency government bailouts.

Estimates suggest that the $770 billion Paycheck Protection Program, for example, ended up costing between $169,000 and $258,000 per job saved—though other researchers have claimed the cost was only about $50,000 per job. The Obama-era American Recovery and Reinvestment Act (ARRA) ended up spending between $50,000 and $112,000 per job created or saved.

The federal bailout of state and local governments during the COVID-19 pandemic has already gone down as one of the most unnecessary responses to that crisis. Now, it might also be remembered as the most wasteful.

Yep, just more and more moronic government waste.

In an interview Monday on The HillTV's Rising, Dr. Anthony Fauci claimed that he "didn't recommend locking anything down" through the course of the COVID-19 pandemic. Fauci is, of course, free to claim what he wants and spin history however he pleases, but he's not entitled to his own facts.
