From the pdf rule book (not the NFHS book)

Extra Point Try = 1 or 2
1. An opportunity to score 1 point from the 3-yard line or 2 points from the 10-yard line shall be granted to the team scoring a touchdown.
• There is a no-run zone within 5 yards of the goal line. You CANNOT run the ball if the ball is lined up within 5 yards of the goal line.
• This includes the 1-point extra point attempt
• Running the ball is acceptable on a 2-point try, unless a penalty moves the spot of the ball within the 5-yard line.

1 - Is the only no-run zone within 5 yards of the goal line?

2 - There is no no-run zone within 5 yards of the first down lines, correct?

3 - Does that apply only for PAT or for 3rd down with 3 to goal also?
 
 
4 - Let's say they go for 1 point.  Dead ball foul, 5 yards against the offense. Replay down.  Ball is now at the 8 yard line.  Can they run?

Based on what you posted, I would say
1. Yes
2. Correct

3. Not exactly sure what you are asking, but even on a PAT, the run rule still applies. 

4. Yes they can now run.

You're right on all of them.  Clarification on 3.  The only context where the 5 yard run rule is mentioned is in the section about PAT.  However the 5 yard run rule applies outside of PAT also.

I contacted Chris Shields to get confirmation.
